David McInnes, MD, MSEd, FAAFP
Program Director
Family MedicineDr. David McInnes has been the Program Director at St. Vincent’s Family Medicine Residency Program and Medical Director of the Family Medicine Center since 2002. Previously, he was the Program Director at Iowa Lutheran Hospital Family Medicine Residency in Des Moines, Iowa, and spent ten years in private practice prior to that position. Dr. McInnes earned his MD from the University of Missouri – Columbia, completed Family Medicine Residency at Iowa Lutheran (University of Iowa) and his MSEd (Medical Education) from the University of Southern California.
He has been actively involved in medical education and healthcare safety and quality improvement for over 25 years. From forming the first physician QI committee at Iowa Lutheran, to his continuing engagement in the development of Patient Centered Medical Home; standardization, quality improvement and patient safety have been a key focus in his career. He is one of the earliest members of the Institute for Healthcare Improvement, served on the board and quality chair for Blue Cross/ Blue Shield, and has spoken at regional and national meetings on teaching safety and quality improvement to resident physicians. In his role as Program Director, Dr. McInnes also teaches and mentors at the hospital bedside and in the Family Medicine Center.
Under Dr. McInnes’ direction, the Residency Program has been the recipient of several grants funding a childhood literacy program, a regional faculty development series, immunization QI projects, and interprofessional education research. His current area of interest is developing future healthcare leaders through a curriculum in health system leadership, advocacy and organizational dynamics, and includes resident experiences in hospital and clinic committees and leadership meetings.
An avid alpine skier, hiker and traveler, he also enjoys exploring all the national parks and historic sites in the Jacksonville area as well as cheering for the Jacksonville Jaguars.

Sonya Dominguez, MD
Associate Program Director
Academic Medicine Fellowship Director
Family MedicineDr. Sonya Dominguez is from Texas and graduated from the Program in Liberal Medical Education at Brown University School of Medicine in 2002. She completed her Family Medicine Residency at St. Vincent's in 2005, where she was Chief Resident, and received the STFM Outstanding Teacher Award.
She serves as the Associate Program Director, Medical Director for St. Vincent's Employee Health, and Past-President of the Medical Staff. Dr. Dominguez is interested in Community Service, Health Equity, and Leadership Development. She started a Reach Out and Read Chapter at the residency in 2003, which has distributed over 20,000 free books to children in Northeast Florida.
She has been awarded several community service awards, including the Brown University/Fleet Fellows Award, the Jacksonville Times Union 40 Under 40 Award, and the Daughters of the American Revolution Community Service Award.
Dr. Dominguez completed fellowships in Faculty Development and Program Director Development and has an academic interest in Emotional Intelligence, Medical Humanities, Diversity and Inclusion, Social Justice, and Telemedicine. She serves on various hospital committees, including the Medical Staff Executive Committee and Chair of the Clinical Competency Committee.
She is married with a son (Devon) in college at UF, a daughter (Gabby) in high school, and a sweet black Lab named Rosie. She enjoys cooking, listening to audiobooks and music, and watching sports or rewatching Ted Lasso or Gilmore Girls.

Jason Largen, MD, FAAFP
Medical Director
Family MedicineDr. Jason Largen is a native of Jacksonville and was actually born in St. Vincent's. He attended the University of North Florida and earned a Bachelor's in Psychology. Prior to medicine, he had a career in IT and business process improvement. Throughout his career, he always felt drawn back to health care, having been a heart surgery patient as a child. When the opportunity arose to return to school, he jumped at the chance and went to the University of South Alabama College of Medicine. He and his wife, a pediatrician now also with St. Vincent’s, were excited to return to their hometown to complete his residency.
As the father of a young child and husband to a pediatrician, he has a special interest in promoting infant health and safety and was the Co-Chair for North East Florida Safe Sleep Task Force for several years. He is also excited to be able to use his prior skills in project management and process improvement to address how we monitor patient safety and outcomes in the clinic. His experience in IT and his enjoyment of all things technology has led to an interest in using Point of Care Ultrasound both in our clinic and in the hospital, particularly in doing Musculoskeletal Ultrasound.
Dr. Largen also continues to provide gender affirming care to the LGBTQ+ community, training residents and other providers in the state to better care for this underserved community. He is part of both local and state organizations working to expand care options and educate providers and the public on the needs of gender affirming care.

Terri Wall, PhD
Assistant Program Director
Director of Behavioral Health
Clinical PsychologyDr. Wall has served as the Director of Behavioral Health for the St. Vincent’s Family Medicine Residency Program since 2009. She earned a BS in Psychology from the University of Florida, a MS in Counseling Psychology from UNF, and a Ph.D. in Counseling Psychology from Penn State University. She has worked in a number of different settings, including academia and private practice. Her interests include integrated primary care, LGBTQ+ health disparities/advocacy, Gender Affirming Care, DEIA, Lifestyle Medicine, and professional wellbeing.
She enjoys consulting with residents, innovative curriculum development with colleagues, and teaching professional collaboration between physicians and behavioral health professionals. In 2013, she developed a post-doctoral psychology Fellowship in integrated care psychology which combines her passion for integrated care and for teaching psychologists and physicians to work side by side in the primary care setting.
She is very active in STFM, serving in the URM mentoring program, the Destigmatizing Mental Health project, and with the Behavioral Health and Family Systems collaborative. She is a certified Balint Group Leader and also serves on the American Balint Society (ABS) Council and as co-chair of the ABS Diversity Committee. She is engaged in the Jacksonville community as a graduate of Leadership Jacksonville (2021), serves on the Board of Operation New Hope (a post-incarceration transition program), and in local committee work in the area of integrated primary care.
Dr. Wall is a Jacksonville native and raised her 4 (now adult) children in Jacksonville with her husband (John). Away from work, she enjoys spending time with her family, walking her dogs on the beach, yoga, tennis, crafting, and reading (almost anything).

Michael Peterson, DO, MPH, FAAFP
Osteopathic Education Director
Family MedicineDr. Peterson completed his medical education at Nova Southeastern University. While at NSU he earned his Master of Public Health and performed research related to the implementation of Patient Centered Medical Homes. He completed his residency training at the St. Vincent’s Family Medicine Center.
He is dually Board Certified in Family Medicine by the American Osteopathic Board of Family Physicians and the American Board of Family Medicine. Dr. Peterson has a particular interest in mentoring and teaching osteopathic medicine and currently serves as the Director of Osteopathic Education. He also serves as the Ascension St. Vincent's Chief of Family Medicine and has achieved the ABFM's Designation of Focused Practice in Hospital Medicine.
Prior to joining the faculty at the Family Medicine Center, Dr. Peterson practiced in a traditional outpatient family medicine office with the Ascension Medical Group. Dr. Peterson has interests in preventative care and improving health outcomes through lifestyle change. In his spare time, he enjoys spending time traveling and hiking with his wife (Melanie) and 3 young boys.

Danielle Carter, MD, FAAFP
Obstetrics and Gynecology Director
Family Medicine & Obstetrics Fellowship DirectorDr. Danielle Carter graduated from Southern Illinois School of Medicine in 2010 and St. Vincent’s Family Medicine Residency in 2013, where she was awarded the STFM Outstanding Resident Teacher Award and the Florida Academy of Family Physicians Exceptional Resident Scholar Award. She went on to complete an Obstetrics Fellowship at the University of Rochester.
She practiced full spectrum Family Medicine in rural Virginia before returning to the faculty in 2016. Dr. Carter enjoys continuing to practice full spectrum Family Medicine, including surgical obstetrics, in the academic setting. She is also very involved in advocacy by serving as the Vice President of the Florida Academy of Family Physicians and as a state delegate to the National Conference of Constituency Leaders and the Congress of Delegates, and has completed a term on the AAFP Board of Directors.
Locally she is involved with the hospital's Perinatal Policy and Safety Group, as well as the Florida Perinatal Quality Collaborative. She also serves as the Medical Director for Ironman Florida and 70.3 Gulf Coast.
When not at work you will likely find Dr. Carter outdoors somewhere. She is an avid endurance athlete who has completed multiple races of varying distances, including Ironman triathlons. She also enjoys rescuing dogs, hiking, and traveling whenever possible.

Lisa Alley, DO, MPH
Family Medicine & Obstetrics
Dr. Lisa Michelle Alley was born and raised in Little Rock, AR and completed her Biochemistry & Molecular Biology Degree with a Minor in Spanish at Centre College in Danville, KY. She returned to her home state where she attended the University of Arkansas for Medical Sciences for her Masters in Public Health & Epidemiology degree. Her medical training was followed by NYIT-College of Osteopathic Medicine in Jonesboro, AR after which she received residency in Family Medicine at St. Vincent's in Jacksonville, FL.
During her residency she completed an OB/Women's Health Track and focused her senior project on Anemia of Pregnancy. This project was presented at several national and local conferences and won 2nd place at the Duval County Medical Society (DCMS) Annual Symposium. After graduating residency, Dr. Alley continued her Obstetrics/Women's Health Fellowship at St Vincent's in 2023 and signed on as FM-OB faculty in 2024.
Lisa enjoys all aspects of family medicine and women's health including general primary care, preventative medicine and lifestyle medicine. She also has interests in reproductive care, breast health, osteoporosis, hormone therapy/menopause, outpatient gynecologic procedures, antenatal/prenatal care, and inpatient obstetrical care including operative and non-operative deliveries.
In her spare time, she enjoys going to the beach, cooking, and traveling as much as possible.

Bashyam Iyengar, MD, MPH, DipABLM, CAQSM

Lifestyle MedicineDr. Bashyam Iyengar is from New Jersey and graduated from Rutgers University with an undergraduate degree in Nutrition. Subsequently, he completed a Masters in Public Health from Rutgers School of Biomedical and Health Sciences (formerly UMDNJ). He received his medical degree from St. George's University School of Medicine. Dr. Iyengar completed his family medicine residency from RWJ Somerset Family Medicine Residency and a sports medicine fellowship from HackensackUMC Mountainside Family Practice Group (formerly Mountainside Family Practice). Afterwards, he spent a year at SUNY Downstate completing a 1-year fellowship in Academic Medicine.
Dr. Iyengar spent time in Northern Washington State serving underserved community centers that serviced many migrant workers and Native American tribe members. Dr. Iyengar moved to Jacksonville where he has worked in many underserved communities, most recently serving as medical director for Sulzbacher Center.
He has many academic interests that include nutrition and lifestyle medicine, working with the underserved and advocacy (most specifically food inequities), sports medicine and musculoskeletal medicine, transition medicine, and environmental medicine. He has served in the past as adjunct teaching faculty for the University of Florida, Mayo Clinic (Jacksonville), and University of North Florida.
He also serves on the board of the North Florida School of Special Education, as well as a board member for Crossover India. He is happily married to Dr. Rita Nathawad (who is a local pediatrician) and they have a son Jeevan.
In his spare time, Dr. Iyengar enjoys spending time with his family, watching basketball, exercising, reading, listening to music, and traveling. He also coaches his son's many activities (soccer, basketball, and flag football).

Julie Kellogg, MD
Pediatric Medicine
Dr. Julie Kellogg grew up in California and graduated from Westmont College in Santa Barbara before attending Indiana University School of Medicine and completing her residency at Naval Medical Center San Diego. She was active duty in the Navy and stationed in Lemoore, California where she was the Chairman of the Pediatric Department. She came to Jacksonville in 2000 and spent 16 years as a Pediatrician at Naval Hospital Jacksonville, first as active duty military and then in a civilian capacity. She was active in many committees and served as Chair of Pediatrics during part of her time there. She was awarded for her work there with the Navy Commendation Medal. She joined St. Vincent’s Family Medicine Center in 2016 and greatly enjoys her work here.
Dr. Kellogg previously served as President of The Northeast Florida Pediatric Society and Vice Chair of the Partnership for Child Health. She continues to love Pediatric Medicine. Some of her areas of interest are asthma, ADHD, breastfeeding education and support, and the effect of racism on child and adolescent health. She is also committed to community outreach and providing care to children who may be medically underserved.
Dr. Kellogg has 4 adult children and a grandbaby, as well as assorted pets. In her free time she adores spending time with her family. She also enjoys exercising, reading, quilting, watching football, and traveling.

D. Robin Potts, MD
Family Medicine
After graduating from the University of South Florida College of Medicine, Dr. Robin Potts attended residency at St. Vincent’s. Upon graduation, she worked as a locum tenens doctor traveling and working around the country but returned to the residency in 1997 because the work environment and location could not be beat.
She enjoys a variety of interests in her career and outside of the office. She does full spectrum Family Medicine and enjoys including some Obstetrics, dermatology procedures, and using her office ultrasound in her practice as well as working with the office EMR to try and make documentation more efficient and improve patient care by including guideline reminders.
She enjoys bicycling, scuba diving, backpacking, RV camping, and boating and has had the good fortune to be able to travel the world doing these activities.

Jennifer Snell, MD
Family Medicine
Lifestyle MedicineDr. Jennifer Rowe Snell is the daughter of a retired Navy hospital corpsman and an elementary school teacher. She was born in Bethesda, Maryland and raised primarily in LaGrange, North Carolina. Jennifer moved to Orange Park, Florida and completed her junior and senior year at Ridgeview High School. She then matriculated to Florida Agricultural and Mechanical University in Tallahassee where she obtained a Bachelor of Science degree in Biology.
While in Tallahassee she also received a Master’s in Biomedical Sciences and MD from Florida State University where she was active in Student National Medical Association (SNMA), Christian Medical Association (CMA), Students Interested in Global Health (SIGH) and participated in two service learning trips to Nicaragua. Jennifer is passionate about providing care to underserved populations, global health, and whole-person care.
She loves singing (she can often be found humming at her desk or anywhere she goes), playing/watching basketball and tennis, spending time with family, reading, and is a nap enthusiast. Dr. Snell is board certified in Family Medicine and Lifestyle Medicine. She is blessed to be back in Jacksonville with her husband and family and very happy to be a part of the St. Vincent's family, living out her dream of being a physician!

Terrance Soldo, DO, FAAFP
Family Medicine
Dr. Soldo earned his DO from the Kirksville College of Osteopathic Medicine in Kirksville, MO in 1990. He did his Family Medicine training at the Naval Hospital, Camp Pendleton, CA and completed the Faculty Development Fellowship in Family Medicine at the University of North Carolina in 1998. Dr. Soldo was a faculty member at the Family Medicine Residency at the U.S. Naval Hospital, NAS Jacksonville, FL for 7 of his previous 10 years, then joined St. Vincent's in 2007.
Dr. Terrence Soldo was the Director of Osteopathic Medical Education from 2007-2023. He is a Fellow of the American Academy of Family Physicians. He has been actively involved in Graduate Medical Education for over 25 years. He spent a total of 36 years in the United States Navy, progressing from a General Duty Hospital Corpsman to Laboratory Technician to Physician Assistant and finally to medical school to become a Family Physician. He served as the first Physician Assistant aboard the USS NIMITZ (CVN-68) during the Iranian hostage crisis in 1979-80 and as a Family Physician with the US Marines during the U.S. invasion of Iraq in 2003 and again in 2004.
Formerly the Director of Osteopathic Training, he is still active in providing training in Osteopathic Manipulative Medicine, both in the clinic and at the hospital bedside. He is very involved in working with the hospital care team of students, residents, clinical pharmacists, case managers, consultants, and others, to identify opportunities for improving hospital care. Under Dr. Soldo's direction, the Residency Program maintained AOA approval of the Osteopathic component while dually accredited and led the program in becoming one of the first programs to obtain Osteopathic Recognition by the ACGME.
Dr. Soldo also served as the Medical Director for the St. Vincent’s Mobile Health Ministry from 2007 until 2024. This involves 5 mobile health units that include dental and vision units as well as the medical and pediatric units. The mobile units, and the staff of physicians, family medicine residents, nurse practitioners, and support staff provide basic primary care medicine to hundreds of underserved and uninsured people in five counties in Northeast Florida. He is currently the Medical Director for the Putnam County Medical Mission, a free clinic with locations in Palatka and Crescent City, Florida.
His greatest accomplishment involves his part in successfully raising five boys into amazing young men with his life partner, and best friend, Deborah.

Sara Kramer-Wallace, ARNP
Family Medicine
Sara Kramer-Wallace grew up in Charleston, S.C. and graduated from The University of Georgia with a Bachelor of Science in Biology. She then went on to pursue her interest in nursing, graduating with Honors from the Medical University of South Carolina with a Bachelor of Science in Nursing. Sara then worked as an ICU RN for 12 years before returning to school to complete her Masters of Science in Nursing as well as a Doctorate of Nursing Practice, both from the Medical University of South Carolina.
Sara is board certified in Family Medicine and licensed in Florida. She is an active member of the American Association of Nurse Practitioners and her professional interests include preventive medicine and patient education.

Christopher Watson, MD, MPH
Family Medicine & Obstetrics
Dr. Watson is a board-certified Family Medicine Physician practicing full scope family medicine including surgical obstetrics in Jacksonville, FL. He is originally from Dothan, Alabama. He is core faculty at the St. Vincent's Family Medicine Program. He also serves as supervising physician for women’s health advanced practice practitioners for a Federally Qualified Health Center (FQHC), Agape Health, in Jacksonville, FL.
He is currently active with the Florida Academy of Family Physicians (FAFP) Justice, Equity, Inclusion and Diversity Task Force as well as the Physician Trustee for the American Academy of Family Physician Foundation. He was also appointed to the Florida Prescription Drug Monitoring Program (PDMP) Foundation Board of Directors where he will work on student educational programs. He is also an active member of the Sigma Pi Lambda Chapter of Alpha Phi Alpha Fraternity, Inc.
Christopher is a Teach For America Alumni as well as a National Health Service (NHS) scholar. As a NHS scholar, he worked in Warner Robins, Georgia at the Houston Healthcare Family Medicine Residency Program where he was responsible for the women’s health and obstetrics curriculum for the program. He was the first family physician to practice obstetrics in that particular healthcare system since the 1970s.
Christopher received his B.S. from the University of Alabama at Birmingham and his MPH from Georgia State University. As an undergraduate Ronald E. McNair Scholar, he conducted research on cancer education and support programs for low-income African American cancer survivors. As a part of his Master’s capstone, he examined strategies to reduce tobacco use in Atlanta. He served as a Mayo Clinic Summer intern and conducted a research project addressing health promotion and intervention for black men.
Between receiving an MPH and enrolling in medical school, Christopher worked as a teacher through Teach for America, where he taught high school science at a low-income community in Atlanta for three years. Christopher graduated from Meharry Medical College in Nashville, TN. He also received a Certificate in Health Policy from the Robert Wood Johnson Foundation Center for Health Policy at Meharry Medical College. During his time at Meharry, Christopher served as the Pre-Alumni President and the Student Observer to the Board of Trustees.
He completed his residency in Family Medicine at Indiana University School of Medicine in Indianapolis, Indiana, and he completed an additional year of training in Family Medicine Obstetrics at Tallahassee Memorial Family Medicine Residency Program in Tallahassee, FL. Christopher has an array of interests in primary care including maternal health, female reproductive health at any age, men’s health, and transgender health. He is interested in creating safe spaces for patients that promote inclusion and diversity.
