    Dr. Sonya Dominguez is from Texas and graduated from the Program in Liberal Medical Education at Brown University School of Medicine in 2002. She completed her Family Medicine Residency at St. Vincent's in 2005, where she was Chief Resident, and received the STFM Outstanding Teacher Award.

    She serves as the Associate Program Director, Medical Director for St. Vincent's Employee Health, and Past-President of the Medical Staff. Dr. Dominguez is interested in Community Service, Health Equity, and Leadership Development. She started a Reach Out and Read Chapter at the residency in 2003, which has distributed over 20,000 free books to children in Northeast Florida.

    She has been awarded several community service awards, including the Brown University/Fleet Fellows Award, the Jacksonville Times Union 40 Under 40 Award, and the Daughters of the American Revolution Community Service Award.

    Dr. Dominguez completed fellowships in Faculty Development and Program Director Development and has an academic interest in Emotional Intelligence, Medical Humanities, Diversity and Inclusion, Social Justice, and Telemedicine. She serves on various hospital committees, including the Medical Staff Executive Committee and Chair of the Clinical Competency Committee.

    She is married with a son (Devon) in college at UF, a daughter (Gabby) in high school, and a sweet black Lab named Rosie. She enjoys cooking, listening to audiobooks and music, and watching sports or rewatching Ted Lasso or Gilmore Girls.

    Dr. Peterson serves as the Associate Program Director and Osteopathic Eduation Director at the Ascension St. Vincent’s Family Medicine Residency Program.

    He completed his medical education at Nova Southeastern University. While at NSU he earned his Master of Public Health and performed research related to the implementation of Patient Centered Medical Homes. He completed his residency training at the St. Vincent’s Family Medicine Center.

    He is dually Board Certified in Family Medicine by the American Osteopathic Board of Family Physicians and the American Board of Family Medicine. Dr. Peterson has a particular interest in mentoring and teaching osteopathic medicine and currently serves as the Director of Osteopathic Education. He also serves as the Ascension St. Vincent's Chief of Family Medicine and has achieved the ABFM's Designation of Focused Practice in Hospital Medicine.

    Prior to joining the faculty at the Family Medicine Center, Dr. Peterson practiced in a traditional outpatient family medicine office with the Ascension Medical Group. Dr. Peterson has interests in preventative care and improving health outcomes through lifestyle change. In his spare time, he enjoys spending time traveling and hiking with his wife (Melanie) and 3 young boys.

  • Terri Wall, PhD

    Dr. Wall serves as Associate Program Director and Director of Behavioral Health and for the St. Vincent’s Family Medicine Residency Program. A Jacksonville native, she holds a Ph.D. in Counseling Psychology from Penn State University and a B.S. from the University of Florida. Dr. Wall’s background spans private practice and academia, fueling her passion for integrated primary care, LGBTQ+ health disparities, and professional well-being. In 2013, she founded a post-doctoral Fellowship to train psychologists and physicians to work side-by-side in integrated primary care settings.

    In addition to her residency leadership, Dr. Wall is a 2021 graduate of Leadership Jacksonville and serves on the STFM Annual Conference Planning Committee. Outside of work, she is an avid reader who enjoys yoga, tennis, crafting, and spending time with her family.

  • David McInnes, MD, MSEd, FAAFP

    Dr. David McInnes was the Program Director St. Vincent’s Family Medicine Residency Program from 2002-2026.

    Under his leadership, the program has grown to what it is today, receiving several grants funding a childhood literacy program, a regional faculty development series, immunization QI projects, and interprofessional education research. His current area of interest is developing future healthcare leaders through a curriculum in health system leadership, advocacy and organizational dynamics, and includes resident experiences in hospital and clinic committees and leadership meetings.

    Previously, he was the Program Director at Iowa Lutheran Hospital Family Medicine Residency in Des Moines, Iowa, and spent ten years in private practice prior to that position. Dr. McInnes earned his MD from the University of Missouri – Columbia, completed Family Medicine Residency at Iowa Lutheran (University of Iowa) and his MSEd (Medical Education) from the University of Southern California.

    He has been actively involved in medical education and healthcare safety and quality improvement for over 25 years. From forming the first physician QI committee at Iowa Lutheran, to his continuing engagement in the development of Patient Centered Medical Home; standardization, quality improvement and patient safety have been a key focus in his career. He is one of the earliest members of the Institute for Healthcare Improvement, served on the board and quality chair for Blue Cross/Blue Shield, and has spoken at regional and national meetings on teaching safety and quality improvement to resident physicians. In his role as Program Director, Dr. McInnes also teaches and mentors at the hospital bedside and in the Family Medicine Center.

    An avid alpine skier, hiker and traveler, he also enjoys exploring all the national parks and historic sites in the Jacksonville area as well as cheering for the Jacksonville Jaguars.

  • Danielle Carter, MD, FAAFP

    Dr. Danielle Carter graduated from Southern Illinois School of Medicine in 2010 and St. Vincent’s Family Medicine Residency in 2013, where she was awarded the STFM Outstanding Resident Teacher Award and the Florida Academy of Family Physicians Exceptional Resident Scholar Award. She went on to complete an Obstetrics Fellowship at the University of Rochester. She practiced full spectrum Family Medicine in rural Virginia before returning to the faculty in 2016.

    Dr. Carter enjoys continuing to practice full spectrum Family Medicine, including surgical obstetrics, in the academic setting. She is also very involved in advocacy by serving as the President of the Florida Academy of Family Physicians (FAFP) as well as a state delegate to the National Conference of Constituency Leaders and the AAFP Congress of Delegates. She has also completed a term on the AAFP Board of Directors. Locally she is involved in with the hospital's Perinatal Policy and Safety Group, is the market representative to Ascension National's Womens' Health Steering Commmittee, and is on the steering committe of the Florida Perinatal Quality Collaborative. She also serves as the Medical Director for Ironman Florida, Jacksonville, and 70.3 Gulf Coast.

    When not at work you will likely find Dr. Carter outdoors somewhere. She is an avid endurance athlete who has completed multiple races of varying distances, including Ironman triathlons. She also enjoys rescuing dogs, hiking, and travelling whenever possible.

    Dr. Lisa Michelle Alley is a clinician-educator and Family Medicine physician specializing in full-spectrum Women’s Health and Lifestyle Medicine. Originally from Little Rock, Arkansas, Dr. Alley’s career is defined by a commitment to bridging the gap between clinical practice and evidence based medicine.

    She earned her Bachelor’s degree in Biochemistry & Molecular Biology from Centre College, followed by a Master of Public Health in Epidemiology from the University of Arkansas for Medical Sciences. This foundational expertise in population health informs her current clinical practice and her approach to preventative care. Dr. Alley received her medical degree from the NYIT College of Osteopathic Medicine and completed her Family Medicine residency at St. Vincent’s in Jacksonville, Florida, where she pursued a specialized OB/Women’s Health track.

    A dedicated researcher, Dr. Alley’s work on "Treating and Preventing Anemia in Pregnancy" has been featured at national conferences and earned second-place honors at the Duval County Medical Society (DCMS) Annual Symposium.She completed her Obstetrics and Women's Health fellowship in 2023 at St. Vincent’s Southside Hospital and joined on as Faculty at Ascension FMC in 2024.

    In her current role, Dr. Alley manages a diverse clinical portfolio, including general primary care, antenatal and reproductive care, inpatient obstetrics (both operative and non-operative deliveries), and outpatient gynecologic procedures. She is a passionate advocate for comprehensive Women's Healthcare including fertility counseling, preventative care, and menopause management.

    Outside of her professional life, Dr. Alley is an avid traveler and cook who enjoys the coastal lifestyle of Jacksonville with her husband and daughter.

  • Jonathan Carbungco, DO, MS

    Dr. Carbungco was born and raised in St. Petersburg, FL, the Sunshine City. He completed his undergraduate degree in Psychology at Florida State University and earned his Doctor of Osteopathic Medicine degree from the Alabama College of Osteopathic Medicine. There, he discovered his love for medical education as a TA and later extended his training to complete a teaching fellowship, earning a master's degree in Adult Education with a focus on curriculum design while serving as junior faculty. While at ACOM, he was inducted into the Gold Humanism Honor Society and received the Sam Cooper Memorial Award for Professionalism.

    Dr. Carbungco completed his family medicine residency at St. Vincent’s in 2025, serving as Chief Resident. Drawn to the field's broad scope and preventive focus, he actively contributed to recruitment and curriculum committees, participated in organized medicine with the FAFP and DCMS, and volunteered in the IRONMAN medical tent. He then completed an Academic Medicine Fellowship specializing in Point of Care Ultrasound and MSK pathology, during which he completed the 1-year STFM POCUS Educator's Curriculum and led the residency's POCUS training.

    Now a full-fledged faculty member, Dr. Carbungco maintains dual board certification by the ABFM and AOBFP with a certification in Osteopathic Manipulative Medicine. His professional interests include primary care, lifestyle medicine, OMM/musculoskeletal medicine, and academic curriculum design. Outside of work, he enjoys listening to podcasts on technology and culture, cooking, trying to keep house plants alive, and binging Star Wars content.

  • Jan Dela Cruz, DO, MMS

    Dr. Dela Cruz was born and raised in Jacksonville, Florida. He majored in Biology at the University of Florida before returning to Jacksonville to teach at his alma mater, Bishop Kenny High School, where he first served as a substitute teacher before teaching Chemistry and Chemistry Honors. He later attended the Lake Erie College of Osteopathic Medicine in Bradenton, Fl where he earned both his Master of Medical Science and Doctor of Osteopathic Medicine degrees.

    After serving as Chief Resident from 2024-2025, Dr. Dela Cruz graduated from the St. Vincent’s Family Medicine Residency Program and was awarded the Jane Lanier Legacy Award, recognizing a graduating third year Family Medicine resident who displayed commitment to improve the community they serve through quality improvement, innovation, and outreach with special attention to the poor and vulnerable.

    He continued his education completing his Academic Medicine Fellowship at St. Vincent's, and contributed to the development of the program’s current didactic and noon conference structure, with a strong emphasis on board-focused education, resident engagement, and learning strategies. He also played an active role in recruitment efforts and enhancing the visiting medical student experience. In addition, he remains dedicated to teaching and mentoring residents while helping lead community outreach initiatives such as Operation HERO, which engages local elementary and middle school students in health and medical education.

    Dr. Dela Cruz now serves as faculty within the residency program, where his interests include medical student education, resident development, recruitment, and community outreach.

    Outside of medicine, his interests include basketball, running, and boxing. He also plays bass guitar in a local cover band. Dr. Dela Cruz is a lifelong and avid fan of the Jacksonville Jaguars and all things Duval.

    Dr. Bashyam Iyengar is from New Jersey and graduated from Rutgers University with an undergraduate degree in Nutrition. Subsequently, he completed a Masters in Public Health from Rutgers School of Biomedical and Health Sciences (formerly UMDNJ). He received his medical degree from St. George's University School of Medicine. Dr. Iyengar completed his family medicine residency from RWJ Somerset Family Medicine Residency and a sports medicine fellowship from HackensackUMC Mountainside Family Practice Group (formerly Mountainside Family Practice). Afterwards, he spent a year at SUNY Downstate completing a 1-year fellowship in Academic Medicine.

    Dr. Iyengar spent time in Northern Washington State serving underserved community centers that serviced many migrant workers and Native American tribe members. Dr. Iyengar moved to Jacksonville where he has worked in many underserved communities, most recently serving as medical director for Sulzbacher Center.

    He has many academic interests that include nutrition and lifestyle medicine, working with the underserved and advocacy (most specifically food inequities), sports medicine and musculoskeletal medicine, transition medicine, and environmental medicine. He has served in the past as adjunct teaching faculty for the University of Florida, Mayo Clinic (Jacksonville), and University of North Florida.

    He also serves on the board of the North Florida School of Special Education, as well as a board member for Crossover India. He is happily married to Dr. Rita Nathawad (who is a local pediatrician) and they have a son Jeevan.

    In his spare time, Dr. Iyengar enjoys spending time with his family, watching basketball, exercising, reading, listening to music, and traveling. He also coaches his son's many activities (soccer, basketball, and flag football).

  • Julie Kellogg, MD

    Dr. Julie Kellogg grew up in California and graduated from Westmont College in Santa Barbara before attending Indiana University School of Medicine and completing her residency at Naval Medical Center San Diego. She was active duty in the Navy and stationed in Lemoore, California where she was the Chairman of the Pediatric Department. She came to Jacksonville in 2000 and spent 16 years as a Pediatrician at Naval Hospital Jacksonville, first as active duty military and then in a civilian capacity. She was active in many committees and served as Chair of Pediatrics during part of her time there. She was awarded for her work there with the Navy Commendation Medal. She joined St. Vincent’s Family Medicine Center in 2016 and greatly enjoys her work here.

    Dr. Kellogg previously served as President of The Northeast Florida Pediatric Society and Vice Chair of the Partnership for Child Health. She continues to love Pediatric Medicine. Some of her areas of interest are asthma, ADHD, breastfeeding education and support, and the effect of racism on child and adolescent health. She is also committed to community outreach and providing care to children who may be medically underserved.

    Dr. Kellogg has 4 adult children and a grandbaby, as well as assorted pets. In her free time she adores spending time with her family. She also enjoys exercising, reading, quilting, watching football, and traveling.

  • Sara Kramer-Wallace, ARNP

    Sara Kramer-Wallace grew up in Charleston, S.C. and graduated from The University of Georgia with a Bachelor of Science in Biology. She then went on to pursue her interest in nursing, graduating with Honors from the Medical University of South Carolina with a Bachelor of Science in Nursing. Sara then worked as an ICU RN for 12 years before returning to school to complete her Masters of Science in Nursing as well as a Doctorate of Nursing Practice, both from the Medical University of South Carolina. Sara is board certified in Family Medicine and licensed in Florida. She is an active member of the American Association of Nurse Practitioners and her professional interests include preventive medicine and patient education.

  • Katherine Napuri, MD

    Dr. Napuri was born and raised in Miami, Florida. As a first-generation Peruvian American, she discovered her passion for medicine early in life and has pursued it with dedication. She earned her medical degree from Trinity School of Medicine and completed residency at UCF in Gainesville, Florida. During residency, she served as Chief Resident and developed a women’s health track for residents interested in Obstetrics. Inspired by her passion for women’s health and obstetric care, Dr. Napuri completed a Surgical Obstetrics Fellowship through Florida State University’s Tallahassee Memorial HealthCare (TMH) program. After completing fellowship, she moved to Jacksonville, Florida, where she joined Ascension St. Vincent's as core FM-OB Faculty, continuing her commitment to medical education, women’s health, OB, and comprehensive patient care. She is also involved in the Family Birth Place Policy and Quality committees. She has two sisters who are also physicians, and play a big part in her journey.

    Outside of medicine, Dr. Napuri enjoys spending time with her family, going to the beach, traveling, CrossFit, and playing pickleball and volleyball. She also loves exploring new restaurants and experiences.

    After graduating from the University of South Florida College of Medicine, Dr. Robin Potts attended residency at St. Vincent’s. Upon graduation, she worked as a locum tenens doctor traveling and working around the country but returned to the residency in 1997 because the work environment and location could not be beat.

    She enjoys a variety of interests in her career and outside of the office. She does full spectrum Family Medicine and enjoys including some Obstetrics, dermatology procedures, and using her office ultrasound in her practice as well as working with the office EMR to try and make documentation more efficient and improve patient care by including guideline reminders.

    She enjoys bicycling, scuba diving, backpacking, RV camping, and boating and has had the good fortune to be able to travel the world doing these activities.

  • Jennifer Snell, MD

    Dr. Jennifer Rowe Snell is the daughter of a retired Navy hospital corpsman and an elementary school teacher. She was born in Bethesda, Maryland and raised primarily in LaGrange, North Carolina. Jennifer moved to Orange Park, Florida and completed her junior and senior year at Ridgeview High School. She then matriculated to Florida Agricultural and Mechanical University in Tallahassee where she obtained a Bachelor of Science degree in Biology.

    While in Tallahassee she also received a Master’s in Biomedical Sciences and MD from Florida State University where she was active in Student National Medical Association (SNMA), Christian Medical Association (CMA), Students Interested in Global Health (SIGH) and participated in two service learning trips to Nicaragua. Jennifer is passionate about providing care to underserved populations, global health, and whole-person care.

    She loves singing (she can often be found humming at her desk or anywhere she goes), playing/watching basketball and tennis, spending time with family, reading, and is a nap enthusiast. Dr. Snell is board certified in Family Medicine and Lifestyle Medicine. She is blessed to be back in Jacksonville with her husband and family and very happy to be a part of the St. Vincent's family, living out her dream of being a physician!

  • Terrance Soldo, DO, FAAFP

    Dr. Terrence Soldo, now a part time member of the faculty, He is the former Director of Osteopathic Medical Education, a position he held from 2007 until 2024. Prior to coming to St. Vincent's he was a faculty member at the Family Medicine Residency at the U.S. Naval Hospital, NAS Jacksonville, FL for 7 of the ten previous years. Dr. Soldo earned his DO from the Kirksville College of Osteopathic Medicine in Kirksville, MO in 1990. He did his Family Medicine training at the Naval Hospital, Camp Pendleton, CA and completed the Faculty Development Fellowship in Family Medicine at the University of North Carolina in 1998. He is a Fellow of the American Academy of Family Physicians.

    He has been actively involved in Graduate Medical Education for over 28 years. He spent a total of 36 years in the United States Navy, progressing from a General Duty Hospital Corpsman, to Laboratory Technician to Physician Assistant and finally to medical school to become a Family Physician. He served as the first Physician Assistant aboard the USS NIMITZ (CVN-68) during the Iranian hostage crisis in 1979-80 and as a Family Physician with the US Marines during the U.S. invasion of Iraq in 2003 and again in 2004.

    In his role as the Director of Osteopathic Medical Educatin Training, Dr. Soldo also teaches and mentors at the hospital bedside and in the Family Medicine Center. He is very involved in working with the hospital care team of students, residents, clinical pharmacists, case managers, consultants, and others, to identify opportunities for improving hospital care.

    Under Dr. Soldo’ direction, the Residency Program maintained AOA approval of the Osteopathic component while being dually accredited and led the program in becoming one of the first programs to obtain Osteopathic Recognition by the ACGME.

    Dr. Soldo also served as the Medical Director for the St. Vincent’s Mobile Health Ministry From 2007 until 2024. This involves 5 mobile health units that include dental and vision units as well as the medical and pediatric units. The mobile units, and the staff of physicians, family medicine residents, nurse practitioners and support staff provide basic primary care medicine to hundreds of underserved and uninsured people in five counties in Northeast Florida.

    Since 2013 Dr Soldo has been one of the Family Medicine faculty making annual trips to China as a member of the International Primary Care Educational Alliance (IPCEA). The organizations goal to advance the recognition and development. of primary care mediine as the foundation required to provide timely and appropriate access to quality and cost effective health care.

    His greatest accomplishment involves his part in successfully raising five boys into amazing young men with his life partner, and best friend, Deborah.

  • James D Stephenson, DO

    Dr. Stephenson, is a board-certified Family Medicine physician with additional board certification in Sports Medicine and Osteopathic Manipulative Treatment. He is a faculty member of the St. Vincent’s Family Medicine Residency Program and provides comprehensive care integrating primary care and musculoskeletal medicine.

    Originally from Raymond, Maine, Dr. Stephenson completed his undergraduate studies at the University of Southern Maine. He later moved to South Florida, where he worked as a personal trainer, met his wife, and earned a master’s degree in biomedical science from Florida Atlantic University. He earned his medical degree from Touro College of Osteopathic Medicine in Middletown, New York, and went on to complete his Family Medicine Residency at St. Vincent’s Medical Center, where he served as Chief Resident, followed by a Sports Medicine Fellowship.

    Dr. Stephenson has served as a team physician at the high school, collegiate, and professional levels. He has expertise in musculoskeletal ultrasound for diagnosis and performs ultrasound-guided procedures as part of his non-operative approach to care. His practice emphasizes individualized, evidence-based treatment plans focused on preventive care, restoring function, and optimizing long-term health and quality of life.

  • Christopher Watson, MD, MPH

    Dr. Watson is a board-certified Family Medicine Physician practicing full scope family medicine including surgical obstetrics in Jacksonville, FL. He is originally from Dothan, Alabama. He is core faculty at the St. Vincent's Family Medicine Program. He also serves as supervising physician for women’s health advanced practice practitioners for a Federally Qualified Health Center (FQHC), Agape Health, in Jacksonville, FL.

    He is currently active with the Florida Academy of Family Physicians (FAFP) Justice, Equity, Inclusion and Diversity Task Force as well as the Physician Trustee for the American Academy of Family Physician Foundation. He was also appointed to the Florida Prescription Drug Monitoring Program (PDMP) Foundation Board of Directors where he will work on student educational programs. He is also an active member of the Sigma Pi Lambda Chapter of Alpha Phi Alpha Fraternity, Inc.

    Christopher is a Teach For America Alumni as well as a National Health Service (NHS) scholar. As a NHS scholar, he worked in Warner Robins, Georgia at the Houston Healthcare Family Medicine Residency Program where he was responsible for the women’s health and obstetrics curriculum for the program. He was the first family physician to practice obstetrics in that particular healthcare system since the 1970s.

    Christopher received his B.S. from the University of Alabama at Birmingham and his MPH from Georgia State University. As an undergraduate Ronald E. McNair Scholar, he conducted research on cancer education and support programs for low-income African American cancer survivors. As a part of his Master’s capstone, he examined strategies to reduce tobacco use in Atlanta. He served as a Mayo Clinic Summer intern and conducted a research project addressing health promotion and intervention for black men.

    Between receiving an MPH and enrolling in medical school, Christopher worked as a teacher through Teach for America, where he taught high school science at a low-income community in Atlanta for three years. Christopher graduated from Meharry Medical College in Nashville, TN. He also received a Certificate in Health Policy from the Robert Wood Johnson Foundation Center for Health Policy at Meharry Medical College. During his time at Meharry, Christopher served as the Pre-Alumni President and the Student Observer to the Board of Trustees.

    He completed his residency in Family Medicine at Indiana University School of Medicine in Indianapolis, Indiana, and he completed an additional year of training in Family Medicine Obstetrics at Tallahassee Memorial Family Medicine Residency Program in Tallahassee, FL. Christopher has an array of interests in primary care including maternal health, female reproductive health at any age, men’s health, and transgender health. He is interested in creating safe spaces for patients that promote inclusion and diversity.
